Thank you for your commitment to becoming a foster carer with Brophy Family and Youth Services. As part of your assessment, we ask you to prepare a short life history - a reflection on your life from childhood through to adulthood. This helps your assessor understand your background, experiences, and the relationships that have shaped you.
Please note: each applicant must complete their own life history. If there are two applicants in your household, please ensure both individuals submit their own form.
You don't need to write a polished account - honest, simple answers are all we need. If any of these questions bring up difficult memories, please reach out to your assessor for support.
