Information disclosure and rights and responsibilities

Thank you for your commitment to becoming a foster carer with Brophy Family and Youth Services. Please read the information below carefully before providing your acknowledgement. Please note: each applicant must complete their own acknowledgement. If there are two applicants in your household, please ensure both individuals submit their own form.

Collection of information disclosure for carers

This is to advise you that Brophy Family and Youth Services will collect and record information about home based carers or foster carers, which will be used to assess your application for, or maintain approval as, a carer with Brophy Family and Youth Services.

The information collected will also be used for the purpose of maintaining your name on a register of carers, which is a requirement under Section 80 of the Children, Youth and Families Act 2005. If you do not provide this information, we may not be able to assess your suitability as an out-of-home carer.

We are required under Section 75 of the Children, Youth and Families Act 2005 to have regard to factors set out in Regulation 9 of the Children, Youth and Families Regulations 2007 before approving a person to act as a foster carer or residential care worker.

This information may be disclosed to the Department of Families, Fairness and Housing and to other persons or organisations providing a service to a child and/or family, such as an out-of-home care service provider. We may also disclose the information to other persons or organisations in the event a concern is raised about the care or safety of a child or young person, or where we are required or authorised to do so by or under law.

Acknowledgement of receipt

I/We have received the "Information for Carers" Notice regarding the collection and recording of information under Sections 75 and 80 of the Children, Youth and Families Act 2005.

I understand that this information may be disclosed to the Department of Families, Fairness and Housing and to other persons or organisations providing a service to a child and/or family, such as an out-of-home care service provider.

I also understand that this information may be disclosed to other persons or organisations in the event a concern is raised about the care or safety of a child or young person, or where it is required or authorised to do so by or under the law.

Consent to share/release information

I hereby give permission for the Department of Families, Fairness and Housing Child Protection to provide a copy of any information they may hold that is relevant to my application to become a foster carer.

Rights and responsibilities of a foster carer

Becoming a carer means being aware of, and abiding by, the responsibilities to Brophy Family and Youth Services, and to the individuals and families with whom you become involved. As a carer, you also have rights, which we as an agency must respect and adhere to.
Rights
• To have support and respect from your supervisor • Not to have unreasonable demands placed upon you • To receive adequate information and a clear duty statement
• To be trusted with confidential information that will help you carry out your role more effectively
• To receive training - initial and ongoing - as well as ongoing supervision
• To be well briefed on the organisation you are working for
• To know who you are accountable to, and have clearly defined channels of communication open to you
• To know why, if you are seen to be unsuitable for a task
Responsibilities
• To be willing to undertake training for the role in order to perform it more effectively
• To have loyalty to the organisation, respect its place in the community and uphold its policies
• To be patient and encourage independence for the child in your care
• To remember that all personal information which you learn whilst caring for a child is strictly confidential
• To respect the privacy of others
• Not to pressure others into accepting your views or standards
• To treat people you are working with as you would like to be treated yourself
• To recognise your own limits and avoid becoming overcommitted
